Open Records Request - Commonwealth Office of the Ombudsman

​​Kentucky residents wishing to obtain a public record(s) through the Open Records Act in the possession of the Auditor of Public Accounts (APA) may submit an open records request directly to our Open Records Custodian: Deputy General Counsel, Maira Gomez. Requests must be made in writing and include the following:

  1. Signature of the requester with his or her name printed on the request;
  2. A description of the records being requested;
  3. A statement that the person making the request is a resident of the Commonwealth, and the manner in which the requester is a resident under KRS 61.870(10)(a) to (f); and
  4. Whether the records requested are for a commercial or noncommercial purpose. Commercial purpose requests will require the payment of a processing fee of $0.10 per page, in advance of receipt of any documents, to reimburse the agency for costs associated in compiling your request. See KRS 61.874(4).

For the citizens’ convenience, the APA has summarized the process and filing procedure into the following two forms, either of which may be used to file such requests with the APA. APA also accepts written requests not using either form, so long as the request meets the requirements of the rules set out above.

Open Records Requests may be submitted by one of the following options:
  1. ​By email to BOTH Auditor.ORR@ky.gov and Maira.Gomez@ky.gov;
  2. By fax to (502)564-2912;
  3. By mail to Auditor of Public Accounts, Attn. Records Custodian, 209 St. Clair Street, Frankfort, KY 40601; or
  4. By hand delivery during office hours 8:00 a.m. to 5:00 p.m. Monday-Friday, except holidays.

Responsive Records may be received by the requester in one of the following methods:
  • ​Paper Copies – fee $0.10 per photocopy
  • Electronic (email) – free if material is under 8 MB. If the responsive material is more than 8 megabytes (“MB”), the documents will be produced on a flash drive. Prior to mailing, the Records Custodian shall provide an invoice to the requester for the production and postage.
